Processes from Referral to Program Placement

When staff, students, or community members recognize students who may need additional support to be successful in an academic environment for social, emotional, physical, and/or academic needs, this process is to be followed. This process aims to identify specific needs and outline past and current interventions to continue to add to the team's knowledge base to support the student better.

This process must be followed before a student is referred to special education, a 504 plan, or a medical plan.

Step 1

An RMT referral form (Attachment A-1) is completed. This form is completed by the recommending person, who could be a staff member, parent, or community member. Once completed, the form is submitted to our Tami Muth, who oversees the RMT process, which concludes with the student being removed from the team’s focus or a referral for SPED, 504, or medical plan.

After receiving the form A-1 Tami Muth will then do the following:

  • Complete the background information form (Attachment A-2); and,

  • Send staff members who interact with the student a data gathering form (Attachment A-3).

These forms are returned to the Counselor/Director of Student Services.

Step 2

Once all the forms have been received or the deadline for form returns has passed, Tami Muth will set up a meeting to discuss the student and their needs. Both the administration and parents/guardians will be invited to this meeting. During the meeting, the agenda below is used to assist the team in identifying what the needs are and what actions will be taken:

  1. Review why the student was referred - Tami Muth

  2. Facilitate a whiteboard activity to identify the following about the student - Tami Muth:

    1. Strengths

    2. Areas of concern

    3. Past and/or current interventions

    4. Define target need

    5. Develop a goal to meet the need:

      1. Name the need

      2. Define what success looks like

      3. List the actions to be done at school and/or home

      4. Develop a data-gathering system

      5. Set a review date six weeks from the start of the actions from Step 3

Following the meeting, Tami Muth will set the review date for the team on their calendar and invite all participants. Tami Muth will also develop the necessary forms or get assistance from a team member. The meeting results will be sent to the staff so everyone is informed and can assist in addressing the student’s needs.

Step 3

At the midpoint of the intervention from Step 2, approximately three weeks, Tami Muth will send out a survey to check on the status of the student’s progress. If the results show that the student is not progressing, the Counselor/Director of Student Services may determine to go to Step 4.

Step 4

At this point, the team reconvenes and uses the agenda below - Counselor/Director of Student Services:

  1. Review of goal and data to be collected - Counselor/Director of Student Services

    1. Strengths of goal

    2. Areas of concern still present, if any

    3. Was success as defined achieved?

      1. If yes, move on to another need if one exists AND determine if intervention needs to continue.

        1. If another goal is created after the first was successful, then this new goal begins back at Step 2.

      2. If no, revise the goal with a new intervention AND set a new review date four weeks from implementing this further intervention.

Communication following the review meeting is essential to all staff so everyone knows the expected actions. The Counselor or Director of Student Services will again develop any necessary forms or documents and may get assistance from another team member if necessary. Also, the Tami Muth will set the next meeting date on their calendar and invite all participants.

Step 5

At the second review meeting, the team will follow the following agenda as facilitated by the Tami Muth:

  1. Review of goal and data to be collected - Counselor/Director of Student Services

    1. Strengths of goal

    2. Areas of concern still present if any

    3. Was success as defined achieved?

      1. If yes, move on to another need if one exists AND determine if intervention needs to continue.

        1. If another goal is created after the first was successful, then this new goal begins back at Step 2.

      2. If no, determine if the need should move on to one of the following referrals:

        1. Special Education Referral (Attachment B)

        2. 504 Plan Referral (Attachment C)

        3. Medical Plan Referral (Attachment D)

Communication following the review meeting is essential to all staff so everyone knows the expected actions. Tami Muth will again develop any necessary forms or documents and may get assistance from another team member if necessary. Also, the Counselor or Director of Student Services will set the next meeting date on their calendar and invite all participants.

If a referral is made to Special Education, 504, or Medical plans, the Counselor or Director of Student Services will complete the referral form and send it to the appropriate personnel:

  • Special Education – School Psychologist

  • 504 Plan - Counselor or Director of Student Services

  • Medical Plan – School Nurse
